Support Bridge is a registered NDIS provider based in Adelaide, South Australia, dedicated to helping individuals achieve their personal and NDIS goals.

With over 20 years of experience and lived experience in the disability sector, Peter and Nazma have developed a trusted, heartfelt approach to care—genuine, consistent, and focused on truly meeting the unique needs of those they support.

Disability support services include in-home support, daily living assistance, personal care, community and social activities, transport, respite, Supported Independent Living (SIL) care, NDIS coordination, and even guitar and drum lessons an in-house music teacher!

At Support Bridge, we pride ourselves on authentic, compassionate care. Our hardworking, reliable team of support workers share the same passion for building trust and stability with individuals, along with giving the primary carer peace of mind that their loved one is safely supported.

Hello from Nazma

FOUNDER AND DIRECTOR

Nazma Yeasmin is a dedicated leader in the human services sector with over 25 years of global experience across disability support, mental health, and community development. As the founder of Support Bridge, Nazma combines a deep personal passion for social advocacy with a sophisticated background in organisational management.

Her extensive qualifications include a Master’s in Political Science, a Diploma in Community Service (Case Management), and specialized certification in NDIS Support Coordination.

Before establishing Support Bridge, Nazma spent 14 years driving community initiatives in Bangladesh and 5 years ensuring safety and compliance in Aviation Security in Australia. This unique intersection of high-level security expertise and frontline care allows her to lead an organisation that is as disciplined and reliable as it is compassionate.

DIRECTOR OF OPERATIONS

With over 13 years of dedicated experience in Disability and Mental Health, Peter leads the operational excellence at Support Bridge. His career is defined by a deep commitment to person-centered care, reinforced by a Certificate IV in Disability and his ongoing Bachelor of Social Work at Flinders University.

Peter’s leadership is backed by a diverse background, including three years as a Supervisor with the Department of Human Services (DHS), where he managed complex teams of 20 staff and supported high-needs clients and their families.

His unique professional history also includes eight years in Aviation Security at Adelaide Airport, where he honed his skills in staff management and seamless program operations.

Hi from Rach

SUPPORT WORKER AND MARKETING MANAGER

Rach has over 10 years of experience with extensive individual support training, clearances and hands-on care for people of all ages – from young people to the elderly. Rach also has over 20 years’ experience in business strategy, marketing and communications.

Outside of work, I love walking, pilates, art, gardening, cooking, exploring markets, beaches, and local events.

I take the time to listen, understand what matters to you, and work with you (and your family) to make sure your support fits your needs – and can adapt as things change. I’m experienced with a wide range of neurological, physical and intellectual conditions including ASD, PDA, ADHD, dementia, diabetes, mental health, multiple sclerosis, and palliative care.
